  • Your personal information may be used and disclosed by those who are involved in your care for providing, coordinating or managing your health care treatment and related services.  This includes consultation with other treatment team members. 

  • We may use and disclose personal information regarding your outpatient care to third party service providers.

  • Personal information may be requested for continuing care, billing, and patient health. The disclosed information may include all or part of your medical record. We will not release any information outside of those purposes and without your authorisation.

  • Promotion of upcoming programs and workshops or mailing lists. You may unsubscribe from our marketing or mailing lists at any time.

Personal information we collect may include health information for the purposes of providing nutrition therapy and courses. This enables us to safely, appropriately and accurately provide our Services and assess the needs of our client in the provision of our Services and to generally assist in optimising the quality of our service.
